Application for License to Operate a Food Service Establishment

A Macoupin County food service establishment license may be obtained in one of the following ways:

  1. Download the application  from our website Word® version or Adobe® pdf version, complete, and send application and fee to Macoupin County Public Health Department.

 

  1. Pick up an application from the environmental office, located within the Macoupin County Public Health Department at 805 N. Broad St. in Carlinville.

 

  1. Call 217-854-3223, ext 246 or 230 and request that an application be mailed to you.
